Introduction

Welcome to Cordlyn, a digital health platform operated by EthosCare Solutions Private Limited (“EthosCare”, “we”, “us”, or “our”).

These Terms and Conditions (“Terms”) govern your access to and use of the Cordlyn mobile application, website, and related digital platforms (collectively, the “Services”).

By using Cordlyn Services, you enter into a legally binding agreement with EthosCare Solutions Private Limited. Please read these Terms carefully.

01Acceptance of Terms

By accessing, registering for, or using the Cordlyn platform, you acknowledge that you have read, understood, and agree to comply with these Terms.

If you are using Cordlyn on behalf of a healthcare facility or organization, you represent that you have authority to bind that organization to these Terms.

Your use is also subject to our Privacy Policy, which explains how we collect and handle your personal and health information.

02Definitions

Cordlyn Platform

The Cordlyn digital health platform, including its mobile applications, web platforms, and related digital tools for managing and accessing personal health records and healthcare services.

Services

All services provided through the Cordlyn platform, including personal health record management, medication reminders, emergency SOS, medical data storage, ABHA integration, and healthcare interactions.

User

Any individual who accesses or uses the Cordlyn platform, including patients, caregivers, healthcare professionals, and visitors.

ABHA

Ayushman Bharat Health Account — India’s national digital health ID issued under the Ayushman Bharat Digital Mission (ABDM) by the National Health Authority (NHA).

Healthcare Provider

Doctors, clinics, hospitals, laboratories, and other licensed healthcare professionals or institutions that interact with users through the Cordlyn platform.

03User Accounts and Registration

Certain Cordlyn services require the creation of a user account. When registering, you agree to:

• Provide accurate, current, and complete information
• Keep your account information updated
• Maintain the confidentiality of your login credentials
• Accept responsibility for all activities that occur under your account
• Notify us immediately if you suspect unauthorized use

Users must be at least 18 years old to create an account independently. Users under 18 may only access the platform under parental or legal guardian supervision.

Cordlyn reserves the right to suspend or terminate accounts if registration information is found to be false or misleading.

04Services Description

Cordlyn provides digital healthcare tools designed to help individuals manage and organize their health information. Services include:

Personal Health Record (PHR) Management

Securely store, organize, and access medical records, prescriptions, diagnostic reports, and other healthcare information.

Medication Reminders

Set personalized reminders for dosage schedules and receive refill alerts to keep treatment consistent.

Emergency SOS

In an emergency, Cordlyn can alert your designated contacts, share your location, and display critical medical information to first responders.

Healthcare Connectivity

Share medical records with healthcare providers and manage healthcare-related activities through the platform with your consent.

Cordlyn continuously improves its services and may modify, add, or remove features at any time. We cannot guarantee uninterrupted service availability.

05Payment Terms

Core health record management features are free for patients. Certain premium features or services may require payment or subscription fees.

Billing

Fees are charged using the payment method you provide at the time of purchase.

Subscription Renewal

Subscriptions may automatically renew unless cancelled before the renewal date.

Taxes

Displayed fees may not include applicable GST or other taxes. Users are responsible for paying any taxes required under Indian law.

Refunds

Refund requests are handled in accordance with Cordlyn’s refund policy. Contact care@ethoscare.solutions for refund queries.

06Data Privacy and Security

Cordlyn values the privacy and security of your personal and health information. Our data practices are governed by our Privacy Policy and the Digital Personal Data Protection Act, 2023 (DPDPA).

By using Cordlyn, you consent to the collection and use of your information as described in the Privacy Policy. We implement reasonable security measures to protect your information. However, no online system can guarantee absolute security.

07Intellectual Property Rights

All content, software, design elements, and functionality available through Cordlyn are owned by EthosCare Solutions Private Limited, its licensors, or authorized partners, and are protected under applicable copyright, trademark, and intellectual property laws.

Without prior written permission, users may not:

• Reproduce or distribute platform content
• Modify or create derivative works
• Publicly display or transmit platform materials
• Copy software or platform features for commercial use

Unauthorized use of Cordlyn intellectual property may result in legal action under Indian law.

08ABDM and Regulatory Compliance

Cordlyn operates in alignment with the Ayushman Bharat Digital Mission (ABDM) guidelines and the National Health Authority’s Health Data Management Policy.

ABHA ID Linking

Users may optionally link their ABHA ID to Cordlyn to access health records from the national health infrastructure. This linking is entirely voluntary and requires your explicit consent.

Consent Management

All health data sharing through the ABDM ecosystem requires your active consent through the Health Information Consent Manager (HI-CM) framework. You can view, manage, and revoke consent at any time.

Data Localization

All health data is stored on servers located within India in compliance with applicable data localization requirements.

Cordlyn is an ABDM-compliant Health Locker. For queries related to ABDM compliance, contact the National Health Authority at abdm.gov.in.
